ProArts Cross-Registration Form
Instructions: Please fill out only one form per course you’d like to enroll in. Be sure to include a second, third, and fourth choice in the event that your preferred course is full. You will be enrolled in one course per application. Please note that you are limited to a maximum of four applications per institution per semester. If you exceed said limit, you may be deemed ineligible for the semester in question. You may first submit two applications, and then may submit another two for consideration if both of your preliminary applications are denied.
Important: A student dropping/withdrawing from a cross-registration course must notify the registrar of their host school and home institution in writing. Failure to do so may result in a failing grade on the student’s record. The amount of credit awarded for cross-registration is decided by the home institution. Cross-registered students are required to follow all policies of their host institution.
